The program’s primary cardiac diagnoses are congestive heart failure (CHF), myocardial infarction (MI), coronary artery bypass graft (CABG), hypertension (acute or unstable), and cerebrovascular accident (CVA/Stroke).
Diabetes Management
The program incorporates the components of structure, process, and outcome as established by the American Diabetic Association (ADA).
Diabetes patient education is done through the implementation of a structured program and well-trained educators providing information and education to patients and their families.
Surgical Recovery Management
The program is designed to give patients back their functional independence and provide maximum clinical outcomes. Our protocols allow patients to recover at home with the reassurance that their post-operative care needs are being met by highly professional skilled nurses and therapists following their physician’s orders and nationally recognized standards of care.
Rehabilitation Management
This program focuses on improving strength and mobility, balance, swallowing and communication disorders, and incontinence issues to promote the patient’s independence. Stroke Management
This program focuses on providing quality care to patients who recently suffered from a stroke by educating them and their caregivers on all aspects of recovery, including preventing future strokes and recognizing early warning signs. This program is based on national standards and evidence-based practice. Patients appropriate for our program include those with a primary diagnosis of stroke or who have a stroke-related disorder such as weakness, paralysis, or dysphagia. Our treatment plan promotes a healthy lifestyle while reducing complications with the after-effects of stroke.
